Documents and Information required to process your Flight order:

1. Exact package/flight description and date of the flight. Jet type, Hotel and/or additional services requested.
   The best way to place your order is to submit it online.
2. ECG full results emailed to us.
        - if you are older than 39 years old ECG stress test results must be forwarded as well!

   Medical Form** has to be filed at least 35 days prior to the actual flight date. Form has to be completed by your doctor and include all medical tests, conclusions and statements from the appropriate physitians who clears you for the Jet Fighter flight(s) in case you had health issues/surgeries in the past.
        ** - Your email client has to be properly setup on this computer with your first and last name in order to send a request to our autoresponder and receive a Medical Form.

3. Copy of your passport emailed or faxed. We need to receive it at least 16 days prior to intended flight date for security clearance at the Airbase.
4. Passport copies of every party coming to Zhukovsky Airbase for security clearance.
5. Correct contact information: Contact phone, mailing address and email address (if applicable).

If you are a US Citizen residing in the United States and need a Travel Visa -- we have to receive passports of every party traveling to Russia no later than 22 days prior to departure date with paid in-full order.
We have to provide paid hotel reservation documents for travel visa to be issued by the Russian Embassy or Consulate.
If you order flight only (no hotel accommodation) -- Visa service is an additional charge.
